Under 18: If you are under 18 you will need parental permission as well as the waiver to participate in the excursion. Be sure to fill out that part on the application.

Please fill out Section 1 and skip to the section that applies to your category of interest. If you would like to sign up for more than one category please fill out the other sections that apply. Once you have filled out the application please send via email either an image/audio or video file pertaining to your selected submission(s). You must sign a waiver and send it along with your file. Please forward all this information to inceptionhightea@gmail.com . Only complete applications with both the promo file and waiver will be considered. In your email please state in the subject line your  Last name_first name_type of category.

***Those selected will have to commit to a rehearsal schedule. This applies to both models and dancers ONLY. We will attempt to have rehearsal schedules take into account requirements such as school and work where possible. This should not be a discouraging factor in the application process.

Only those selected for the event will be contacted. We would love to include everyone but unfortunately there are limited spots available.
